[*] INOC-DBA and ITAD notes

I got the ITAD working quite easily, the only trick was RTFMing and 
allowing the promiscuous relaying.  So now I'm 269423*262 and can also 
dial other ITAD peers with the 012 prefix.

I had also mentioned a SIP server that lets you quickly dial other NOCs.  
It's called the Inter-NOC Dial-by-ASN (INOC-DBA) at 
https://www.pch.net/inoc-dba/docs/inoc-dba-sip-conf-faq.txt

Still working on setting up the latter, since my * box is behind a NAT gw.  
Phone numbers there are of the form ASN*ext, so I'd be something like 
27265*999
